Initial Teacher Two/Three-Year Checklist (docx)
Applicants who are new to the profession or new to Alaska use the standard application, the program verification form, and this checklist to apply.
Initial Teacher Program Enrollment Checklist (docx)
Applicants holding a bachelor’s degree and have enrolled in a state-approved teacher preparation program use the standard application, the proof of program enrollment form, and this checklist to apply. Applicants must be sponsored by an Alaska school district.
Second Initial Teacher Application & Checklist (docx)
Applicants who have previously held an Initial teaching certificate but who have not satisfied the two years of teaching experience requirement for the Professional teaching certificate use this application and checklist to apply.
Professional Out-of-State Teacher Checklist (docx)
Applicants who hold a valid regular out-of-state teaching certificate and have two years of certified, full-time teaching experience use the standard application and this checklist to apply. A regular teacher certificate is a renewable teaching license issued by another state or jurisdiction that requires at least a bachelor’s degree and the completion of a state-approved teacher preparation program including a student teaching or clinical experience. A regular teacher certificate does not include a restricted, limited, probationary, provisional, emergency, substitute, student teaching or temporary certificate. When your contract for instructional services begins in an Alaska School District, the out-of-state, regular certificate must still be valid and current.

Applicants who are eligible for the reinstatement of a Professional or Master Teacher certificate cannot apply for the Initial or the Second Initial teacher certificate.

Limited Type-I Checklist (docx)
For applicants nominated by their school districts in completing the statutory requirements for full teacher certification. The certificate coordinates the efforts of the applicant, the nominating district, and an approved teacher preparation program toward the applicant’s completion of a baccalaureate degree, an approved teacher preparation program, and passage of the Praxis I examination within a four-year time frame beginning with the 2004-2005 school year. By district request ONLY. A Standard Application must be completed and submitted with this checklist, as well as a District Request and Assurance form completed by the sponsoring Alaska School District.

For applicants holding a bachelor’s degree, fluent in a foreign language and will be working in a language immersion school. By district request ONLY. Special Education Waiver Application & Checklist (docx)

Emergency Teacher or Administrator Certification Application & Checklist (docx)
For applicants who are unable to satisfy one or more of the following requirements of an Initial Teaching Certificate or an Initial Type B Administrative Certificate (Provisional): completion or enrollment in an approved preparation program, basic competency or content area exam, or completion of a bachelor’s or master’s degree. By district request ONLY. The application and checklist must be completed and submitted with supporting documents; including the District Request and Assurance—Emergency Certificate Form completed by the sponsoring Alaska School District.

Add AK Reads Act Endorsement (docx)

Add/Remove Teaching Endorsement (docx)
For holders of a teaching certificate who wish to add or remove, one or more endorsements. (An individual who applies for an Initial teacher certificate with a valid out-of-state certificate must have an Institutional or State recommendation on file with the Teacher Certification Office to be eligible to add an endorsement.)

Background Check Form (docx)
Submit one fingerprint card prior to submitting your completed application. If the certification application is received within 90 days of the background check submission, the application packet will not require an additional fingerprint card.
Duplicate Certificate/Postcard Request (docx)
Request a duplicate copy of your teaching, administrative, or special services certificate. You may also use this form to request a duplicate copy of your yellow postcard. Only an applicant who is required to submit a fingerprint card at the time of application is sent a yellow postcard. Postcards are not sent to student teachers and individuals applying for lifetime certificates.
Expired Initial Certificate Continuation Request (docx)
If the unmet requirements for an expired Initial certificate have been satisfied, an individual may use this form to request the continuation of the certificate. Only expired Initial certificates that were issued less than three years ago are eligible for a continuation request.
Initial Teacher Extension Request Form (docx)
If the requirements for the extension of a current Initial certificate have been satisfied, an individual may use this form to request the extension of the certificate’s expiration date. The requirements must be submitted to the Teacher Certification Office on or before to the expiration date of the certificate.
Mandatory Training Record Form (docx)
Submit this form with your certification application to document the four mandatory training required beginning June 30, 2017.
Name/Address Change Form (docx)
Notify the Teacher Certification Office if your name and/or address has changed. You can change your address electronically.
Non-academic Credit Pre-approval for Sponsors (docx)
Sponsors of institutes or workshops may request pre-approval for the participants of their institutes or workshops to earn non-academic renewal/reinstatement credit. Only institutes or workshops of at least 15 contact hours in duration are eligible for non-academic credit.
Non-academic Credit Pre-approval for Travel (docx)
Participants of an organized, sponsored educational travel event related to their endorsement area(s) may request pre-approval for the event to count toward non-academic renewal/reinstatement credit. Only organized, sponsored educational travel events that are at least three weeks in duration are eligible for non-academic credit.
Non-academic Credit Pre-approval for Workshops (docx)
Participants of institutes or workshops may request pre-approval for an institutes or workshops related to their endorsement area(s) to count as non-academic renewal/reinstatement credit. Only institutes or workshops of at least 15 contact hours in duration are eligible for non-academic credit.
Proof of Program Enrollment Form (docx)
Provide form and list of endorsements to the university or college where you are enrolled in an approved teacher preparation program. The university or college should return the completed form directly to you.
Reinstatement from a Retired/Lifetime Certificate Application & Checklist
Holders of a Retired/Lifetime certificate may reinstate their regular certificate within five years of the regular certificate’s expiration date. Six semester hours or nine quarter hours of credit must have been earned from a regionally accredited university within five years of the reinstatement application.
Renewal Credit Pre-approval Request Form (docx)
Individuals preparing to take renewal credit through a university or an organization approved to offer continuing education units may request pre-approval of the credit. This request form is only necessary if the credit to be earned does not have a clear connection to the individual's employment that requires a teacher, administrative, or special services certificate, the individual's endorsement areas, or a program that leads to an additional endorsement under 4 AAC 12.395. Only a continuing education unit of at least 15 contact hours in duration may be used for renewal credit.

Student teachers serving in Alaska public schools must receive authorization from the Teacher Certification Office. To apply for authorization, submit a complete student teacher authorization form and a fingerprint card prior to your planned student teaching experience.
